The Truth About CO₂ Laser: Not All CO₂ Treatments Are the Same

5 min read

CO₂ laser is regularly described as the gold standard in skin resurfacing—but simply seeing “CO₂ laser” on a treatment menu does not tell the full story.

Different CO₂ systems can deliver energy in very different ways. The available pulse shapes, treatment depths, fractional patterns, scanning technology and level of control can all affect the type of treatment that can be performed.

Some systems provide basic fractional resurfacing. Others can move from a superficial CoolPeel through to advanced fractional and fully ablative treatment.

The laser platform used at Laser & Skin Belfast was selected because it offers that complete range.

CO₂ Has Changed

Traditional CO₂ resurfacing became known for impressive results, but also for an aggressive recovery involving significant redness, swelling, peeling and prolonged healing.

That reputation still makes some patients nervous.

Modern CO₂ technology allows the energy to be delivered much more precisely. Instead of automatically removing the entire surface of the skin, the laser can create thousands of microscopic treatment points while leaving surrounding tissue intact.

The untreated skin between these points supports healing, making it possible to achieve genuine resurfacing with considerably more control over recovery.

CO₂ has not become weaker. It has become more precise.

Fractional Does Not Mean Ineffective

Fractional treatment does not simply mean “a lighter version” of CO₂.

It means the practitioner can control the percentage of skin treated during each session. The spacing, depth, density and energy of the microscopic treatment points can be adjusted according to the concern and desired recovery.

A light fractional treatment can improve pores, fine lines and surface texture with minimal interruption to everyday life.

A deeper, denser fractional treatment can create a much stronger response for established wrinkles, acne scarring and advanced skin damage.

The important question is not whether a CO₂ treatment is fractional. It is how much control the system provides over that fractional treatment.

What Is DOT Technology?

The CO₂ system at Laser & Skin Belfast uses advanced DOT technology.

Each DOT is a microscopic treatment point that creates controlled ablation within the skin. Thousands of these points can be delivered across the treatment area in a carefully selected pattern.

The practitioner can adjust:

  • The distance between the DOTs

  • The density of treatment

  • The depth of each treatment point

  • The size and shape of the scanning area

  • The time the laser remains on each DOT

  • The amount of ablation and thermal stimulation

  • The order in which the treatment points are delivered

This level of control allows treatment to be tailored much more precisely than a fixed-strength laser procedure.

Delicate areas can be treated differently from thicker skin. Deep scars can be treated differently from superficial lines. A patient wanting minimal downtime can receive a completely different treatment from someone seeking the greatest possible correction.

Why SmartStack Matters

SmartStack is one of the most important features of the technology.

It allows between one and five consecutive pulses to be delivered into the same microscopic DOT. This gives the practitioner precise control over how deeply the treatment penetrates and how much thermal effect is created.

Instead of relying on one longer, uncontrolled burst of energy, SmartStack divides the treatment into a series of carefully delivered pulses.

The tissue is given time to cool between those pulses, helping to limit unnecessary lateral heat while allowing greater depth when required.

This is particularly valuable when treating:

  • Acne scarring

  • Deeper wrinkles

  • Lines around the mouth

  • Ageing around the eyes

  • Surgical or traumatic scars

  • Delicate treatment areas

  • Patients requiring a carefully controlled recovery

SmartStack means the treatment can be made deeper without simply making it hotter.

That distinction matters.

The Shape of the Pulse Changes the Treatment

The same CO₂ wavelength can create very different effects depending on how its energy is delivered.

Advanced pulse-shape technology allows the laser to prioritise superficial ablation, deeper vaporisation or a stronger thermal and collagen-remodelling effect.

A very short, high-peak pulse can remove superficial tissue quickly while limiting the amount of residual heat left behind. This is the principle behind CoolPeel.

Alternative pulse shapes can be selected where a greater thermal effect or deeper correction is needed.

This means the practitioner is not limited to turning the power up or down. The actual shape and behaviour of the pulse can be changed to suit the treatment.

That is what separates an advanced CO₂ platform from a basic resurfacing laser.

CoolPeel Is Genuine CO₂ Resurfacing

CoolPeel is sometimes mistaken for a completely different type of laser because its recovery can be so much lighter.

It is still genuine fractional CO₂.

The difference lies in the way the pulse is delivered. The energy is released rapidly, creating controlled superficial ablation while limiting unnecessary heat in the surrounding tissue.

This makes CoolPeel suitable for concerns such as:

  • Fine lines

  • Enlarged pores

  • Rough texture

  • Early sun damage

  • Crepey skin

  • General skin renewal

Patients may experience redness, warmth, dryness or a rough texture, but the recovery is considerably more manageable than traditional aggressive CO₂ resurfacing.

CoolPeel provides a genuine resurfacing treatment for patients who want more than a facial but are not ready for deep CO₂.

The Same Laser Can Go Much Deeper

The real advantage of this technology is that it is not limited to CoolPeel.

The treatment depth, density, pulse shape and SmartStack level can be increased for patients with more significant concerns.

This allows the same CO₂ platform to perform:

  • Corrective fractional resurfacing

  • Deep treatment for acne scarring

  • Advanced wrinkle resurfacing

  • Deep peri-oral treatment

  • Non-surgical blepharoplasty

  • Full-face deep resurfacing

  • Treatment of surgical and traumatic scars

  • Resurfacing of the neck and décolletage

  • Treatment of crepey skin on the body

Patients are therefore not being fitted into one standard CO₂ procedure. The procedure is designed around the patient.

Why Cheaper CO₂ Is Not Necessarily the Same Treatment

Two clinics may both advertise “fractional CO₂ laser,” but that does not mean the technology, practitioner experience or treatment capability is equivalent.

A lower price may reflect a shorter appointment, a basic treatment protocol or a system offering fewer controls over pulse delivery, depth and scanning.

The name of the laser category alone cannot tell patients:

  • How precisely the depth can be controlled

  • Whether the pulse shape can be changed

  • Whether the system can deliver superficial and deep treatments

  • How treatment density is selected

  • Whether advanced stacking technology is available

  • Who performs deeper procedures

  • What level of clinical oversight is provided

  • Whether the treatment is genuinely tailored

Patients should not compare CO₂ treatments on price alone. They should compare the technology, treatment intensity, practitioner and expected result.

Why Practitioner Experience Still Matters

Advanced technology creates options, but those options must be selected properly.

The practitioner needs to understand how the skin will respond to changes in depth, density, pulse shape, heat and fractional coverage. Stronger settings are not automatically better, and light settings should not be used when they are unlikely to address the patient’s concern.

At Laser & Skin Belfast, CoolPeel and mild-to-moderate resurfacing are performed by trained clinic practitioners. Deep resurfacing procedures for moderate-to-severe wrinkles and advanced concerns are performed by Dr Patrick, who also oversees patient care across the clinic’s CO₂ treatments.

This allows patients to be directed to the appropriate treatment level rather than simply being offered the same procedure regardless of their concerns.

The Truth About CO₂ Laser

CO₂ remains the gold standard because it can create a level of genuine resurfacing that many lighter technologies are attempting to imitate.

But not every CO₂ laser offers the same control, and not every advertised CO₂ treatment produces the same effect.

The technology used at Laser & Skin Belfast combines:

  • Genuine 10,600 nm CO₂ resurfacing

  • Adjustable DOT spacing and density

  • SmartStack depth control

  • Multiple pulse shapes

  • Control over ablation and thermal stimulation

  • Superficial CoolPeel treatments

  • Deep fractional resurfacing

  • Fully ablative treatment capability

This is not one treatment sold at several different strengths.

It is a complete resurfacing platform capable of treating everything from early textural changes to deeper wrinkles and established acne scarring.

That is the real difference—and why the technology behind a CO₂ treatment matters just as much as the words “CO₂ laser.”
